2.1 Telemedicine Services
Elixos provides telemedicine services to patients. By using our telemedicine services, you acknowledge that:
Telemedicine consultations may have limitations compared to in-person visits
You will provide accurate and complete health information
You are responsible for following up on any recommended tests or treatments
In case of emergency, you should call 911 or go to the nearest emergency room
2.2 Physician-Patient Relationship
A physician-patient relationship is established when you schedule and complete a consultation with Dr. Liu. This relationship is governed by professional medical standards and applicable laws.
2.3 Not Emergency Services
IMPORTANT: Elixos does not provide emergency medical services. If you are experiencing a medical emergency, call 911 immediately or go to your nearest emergency room. Do not use our telemedicine services for emergency situations.
3. Cash-Pay Practice
3.1 Payment Policy
Elixos is a cash-pay practice. We do not accept insurance or bill insurance companies on your behalf. You are responsible for full payment at the time of service.
3.2 Insurance Reimbursement
While we do not bill insurance, we can provide you with a superbill (itemized receipt) that you may submit to your insurance company for possible reimbursement. We make no guarantees regarding insurance reimbursement.
3.3 Fees and Payment
All fees are due at the time of service
Payment must be made via accepted payment methods
Prices are subject to change with notice
No refunds for completed services unless required by law
4. Appointment Policy
4.1 Scheduling
Appointments must be scheduled through our official booking system. You are responsible for arriving on time for telemedicine appointments.
4.2 Cancellation and No-Show Policy
Cancellations must be made at least 24 hours in advance
Late cancellations (less than 24 hours) may be subject to a cancellation fee
No-shows may be charged the full appointment fee
Repeated no-shows may result in discharge from the practice
5. Patient Responsibilities
As a patient, you agree to:
Provide accurate and complete medical information
Follow treatment recommendations or inform your provider if you choose not to
Ask questions if you don't understand something
Notify us of any adverse reactions or concerns
Respect staff and maintain appropriate communication
Pay for services in a timely manner
6. Prescriptions and Medications
